Reported how Ohio State University student Andrew Polakowski, 18, was killed after he was pinned by an elevator. Andrew was the last person in a group of students to enter the elevator on the third floor about 11:30 p.m. when it unexpectedly began to descend with the doors open.

Reported that there were 24 people on the elevator at the time of the accident. Given that the elevator’s capacity was 2,500 pounds, the elevator was almost certainly overloaded, as each student would have had to weigh 104 pounds at the most to remain within the limit.

The article addressed the following uncertainies:

"While we do not know why Polakowski tried to leave the elevator after it began to descend, nor why so many students were on the elevator, we do know how an elevator is supposed to operate - and it is not with the doors open. So the most glaring question about the accident is: Why was this elevator malfunctioning and able to operate with the doors wide open? Even if the elevator was filled above capacity, are there no safety protocols to prevent exactly what happened if a passenger attempts to exit an overloaded elevator?"

Reported that students piling onto an already crowded elevator car at Ohio State University may have encouraged still more people to enter.  A report issued as part of an investigation found that students didn't intend to overload the elevator car but quickly began to fill the car as more and more people arrived. "Some people may have been holding the doors open to allow more people to enter, and were possibly encouraging more to step onto the elevator," the report said.

Freshman Andrew Polakowski, 18, was fatally injured when he was pinned between the top of the elevator doors and the third floor.  The report said Polakowski had been drinking before the accident but didn't appear intoxicated. Test results showed Polakowski had only a trace amount of alcohol in his system, not enough to impair his abilities.
